In completing this form on behalf of the above team I agree to abide by the rules of the Busselton Masters Competion and the organising committee decisons. I agree not to hold the organisers liable for any damage in respect to any claims made in relation to events held as part or in conjunction with the Busselton Masters Carnival.

I aknowledge there will be no refund given for withdrawal of a team.

Clubs with multiple teams must nominate each team individually.
